The FCC Webmaster has created an excellent information page that contains the majority of the forms required in most interactions with the FCC. The page is located at http://www.fcc.gov/formpage.html. Next to each form name you will find a description of the use of the form, the name of the file, and the last edition date. The forms are generally available as Adobe Portable Document Files (pdf files). So you will need a viewer to see them. The company that makes pdf files offers a free version of the viewer. The company is Adobe, the name of the viewer is Adobe Acrobat Viewer, and can be found at http://www.adobe.com.

Frequently Used FCC Forms In Amateur Radio

Form 605 - For new license applications and upgrades, license renewals, address, name, and systematic call sign changes.
Form 159 - To be submitted with a vanity call sign fee remittance.
Form 606 - Taxpayer Identification Number (TIN) Registration Form.
